‘I Couldn’t Even Unseal My Eyes. That’s The Extent of the Swelling I Was’

Amanda Preisinger is nervous about her child’s impending 13th birthday celebration. Not for the typical causes associated with a home packed of loud adolescent kids, but because it’s the initial occasion she will reveal her new face to friends and relatives. “Obviously I’m going to inform all guests as they arrive, ‘Just so you know, this is not how I look,’” says the thirty-year-old property agent from Southern Florida.

How she looks is, admittedly, a little startling – her face swollen and preternaturally lifted, as though secured by heavy-duty tape. Her altered – and she’s eager to emphasize, temporary – look is the outcome of half a dozen cosmetic procedures, such as an minimally invasive facelift, conducted by a doctor in Istanbul, Turkey, last month. “My spouse teared up when he saw me for the initial time because I couldn’t even open my eyes. That’s how swollen I was,” she explains to me via video call from her house. “I needed to inform him: ‘Babe, I’m fine, I’m not in pain. I just look like someone jumped me.’”

Growing Trend of Early Facelifts

According to cosmetic specialists, Preisinger is one of a rising count of people choosing to undergo a facelift in their 20s and 30s – well over a ten years prior to typical surgeons’ typical patient age range of forty to sixty. (Though most surgeons are keen to emphasise the professional guideline of: “We address genetics, not years.”) “I have individuals in their late twenties asking for facelifts,” says London-based cosmetic surgeon Georgios Orfaniotis, a former NHS consultant who focuses on the facial area. “It’s a very significant procedure, and I’m a somewhat worried when I see individuals choosing it as a personal preference.”

Explaining the Rhytidectomy Surgery

A facelift, also known as a facelift, elevates the ligaments in the face that start to sag as we grow older. “Our faces are structured a bit like onion layers,” says Kent-based face specialist Marc Pacifico. “Skin and fat on the surface, then a layer of supportive tissue known as the superficial musculoaponeurotic system. Consider the SMAS as the soft tissue skeleton of the face. And that’s what causes aging; that is what becomes lax and pulls the dermis downward with it.”

Once the preserve of affluent seniors, “filler fatigue” – when excessive use of dermal fillers stretches the face and leads to laxity in the dermis – alongside the widespread use of weight-loss drugs, low-cost medical tourism, and the development of new, celebrity-endorsed operative methods are driving a different type of customer towards this major operation. Reports indicate an 8% increase in facial lifts over the last year in the United Kingdom; while a survey found that the portion of younger facelift patients is increasing, with 32% of facelifts now conducted for patients aged 35-55.

“People are asking now to look like the best version of themselves and obviously the techniques are evolving,” says Orfaniotis. At present, the operation du jour is the deep plane facelift, a method evangelised by figures including Kardashian matriarch Kris Jenner, to fashion designer Marc Jacobs.

Advanced Lifting Method

Where a conventional rhytidectomy entails lifting the SMAS, the comprehensive lift releases the underlying structures underneath, allowing doctors to restructure the face by repositioning the underlying layers, and preventing the taut, pulled look sometimes wrought by older methods. “We avoid placing tension on the skin because we’re targeting deeper, to the deeper tissue,” says prestigious London cosmetic doctor Rajiv Grover, who focuses on the deep plane technique. “It is the underlying layers that is repositioned. The dermis follows along as a secondary element.” Lifting the entire soft tissue layer as a unified block allows for a authentic-appearing and more durable outcome. It also means that the procedure is no longer being used for the primary goal of anti-ageing – or rejuvenation, as the cosmetic field terms it – but for “enhancement” and facial restructuring, too. “We’re getting many inquiries from patients aged 28 to 35 for this purpose,” says Grover.

Individual Experience

Preisinger had not intended on getting a facial lift – at minimum not at the thirty years old. But years of using hyaluronic acid dermal fillers in an effort at “facial balancing” implied that by the time she entered her late 20s, she was “botched”. “I didn’t do this because I wanted to look young,” she clarifies. “I did it because the injectable harmed my face. I regret ever using it. If I had avoided the injectable, I don’t believe I would be in this situation right now.”

If dermal fillers completely disappear has historically been a point of contention in the cosmetic field. “Studies have indicated that not only do they rarely completely dissolve,” states Grover, “but they move and can obstruct drainage pathways. One of the contributors to what we term ‘pillow face’ is also the fact that in a sense, the face gets somewhat fluid-filled because its ability to remove bodily fluid has been diminished.” Although studies into the topic is preliminary, warnings on injectables’ possible effects on the lymphatic system have been issued, and additional studies initiated. An esteemed facial plastic surgeon, speaking on the condition of anonymity, says he would never use injectables in a client because of the dangers they pose. “Many doctors avoid discussing this, because the companies who make injectables can be pretty aggressive.” He’s heard stories, he adds, of plastic surgeons being landed with legal actions after expressing worries.

Choice and Surgery

For Preisinger, once she started injecting filler, she felt as if she needed to continue replenishing it – especially as it began moving to other parts of her face. She was only twenty-eight when a local plastic surgeon in Florida proposed that a face and neck lift might be what she needed to exit the cycle of treatments. Two years later, she found herself selecting from a selection of recommended hotels in Istanbul, sent to her by the clinic assistant at her surgeon’s office.

Originally, Preisinger had scheduled an upper and lower blepharoplasty – better known as eyelid surgery – but her surgeon advised her that a facial lift was the correct option for the looseness she was finding in her face. She arranged a brow lift, a mid-facelift and a jawline surgery. The day before her surgery, one day after she’d arrived solo in Turkey, she decided to add a lip lift. “Then he said, ‘We’re going to elevate the under-eye bags.’ Then he said, ‘I think if we remove some buccal fat, it will sculpt your face.’ So I ended up adding two more surgeries,” she recalls. She paid $22,000 (sixteen thousand five hundred pounds) for the half-day operation, including a three-day|
